Partners In Care/Continuum Health is seeking a Clinical Data Specialist to join its exciting Clinical data abstraction team. The Clinical Data Specialist is a high- performer who is responsible for the collection of data to improve patient care, process of care and outcomes.


Responsibilities will include monitoring of data collection, data quality, and dataset preparation. Data management responsibilities will also include assistance in developing and providing support for databases, creating and revising reports, implementing data management plans, reviewing queries and reports to ensure data integrity, and communicating with staff at multiple sites related to database support and discrepancy resolution.


This is a staff position within the ACO and is responsible for working collaboratively with Performance Improvement and Information Technology / Business Intelligence departments and the physician practices to achieve ACO quality measure goals and objectives.  Data review, abstraction of data from various medical record modalities and data validation is required as well as participation in quality related performance improvement activities. 

Essential Duties


  1. Responsible for reviewing medical records, and abstracting clinical data elements for quality measures aligned with ACO contractual criteria.
  2. Use system derived data to identify and intervene prospectively for potential quality gaps, process variation, unwarranted use patterns
  3. Enters quality measure data into appropriate quality databases and interfaces to facilitate reporting.
  4. Responsible for learning/maintaining current knowledge of ACO quality program requirements.
  5. Contributes to business requirement development for standardized reports focused on the successful delivery of ACO outcomes measures. Considers data derived from Health Catalyst, Epic, ACO generated reports (commercial, direct to employer or government).
  6. Completes electronic health record (EHR) / chart review, data entry, quality measure, scanning results and other reporting requirements by deadlines as stipulated by ACO.
  7. Ability to navigate and understand clinical data repositories including various EHR’s.
  8. Assists the Quality Data Analysts with reports that aggregate quality program data to facilitate performance improvement initiatives within the physician partners network, ACO, etc.
  9. Serve as liaison with technology vendors and test new system solutions and interfaces for data collection and management systems and electronic medical records; guide development of reports, to collect clinical information
  10. Query databases for data accuracy and coordinate with clinical staff to address discrepancies and maintain data integrity at various sites; review/confirm resolution of data discrepancies; compile and submit to physicians, stakeholders, and sponsors monthly data event reports; ensure security of databases; prepare and submit periodic reports to various sponsors.
  11. Create data sets from internal and external data sources, perform statistical reporting and analyses under the guidance of the ACO leadership
  12. Monitoring milestones, create and present reports to ACO team and sponsors.
  13. Support and communicate with PHM physicians, and other coordinators as needed for successful clinical results.
  14. Local travel to network practices may be required, 30 to 50%
  15. Performing additional duties as assigned
  16. Adhering to all Continuum Health guidelines.




  • Minimum of 1-3 years healthcare or clinical experience required.
  • Experience in working with an Electronic Health Record.
  • Excellent oral and written communication skills.
  • Performance Improvement and Quality Management experience a plus.
  • Prior experience in clinical quality measure data abstraction a plus.



  • Demonstrates understanding of Ambulatory/Practice-Based clinical environments and workflows.
  • Proficiency in database, spreadsheet, and word processing software applications is helpful. Microsoft Office, excel proficiency required.
  • Familiar with NCQA, CMS, IHA, HEDIS, AHRQ resources, quality measures and performance improvement methodologies.
  • Preference given to candidates with demonstrated knowledge of report writing.

  • Bachelor degree in health, science, clinical data management, applied mathematics, statistics, informatics, or computer science, a plus.
