Partners In Care/Continuum Health is seeking a Clinical Data Specialist to join its exciting Clinical data abstraction team. The Clinical Data Specialist is a high- performer who is responsible for the collection of data to improve patient care, process of care and outcomes.
Responsibilities will include monitoring of data collection, data quality, and dataset preparation. Data management responsibilities will also include assistance in developing and providing support for databases, creating and revising reports, implementing data management plans, reviewing queries and reports to ensure data integrity, and communicating with staff at multiple sites related to database support and discrepancy resolution.
This is a staff position within the ACO and is responsible for working collaboratively with Performance Improvement and Information Technology / Business Intelligence departments and the physician practices to achieve ACO quality measure goals and objectives. Data review, abstraction of data from various medical record modalities and data validation is required as well as participation in quality related performance improvement activities.
