General Summary: The Practice Transformation Coordinator will project manage healthcare delivery transformation with healthcare providers and staff. Through a combination of on-site visits, remote visits, e-mail, and phone contact, the Coordinator will support practices with redesigning value-based care delivery workflows that allow the practice to implement care on a population level, improve patient experience, quality of care, and lower cost of care.


Project Management

  • Develop and maintain project plans
  • Proactively manage issues and risks
  • Facilitate practice meetings
  • Communicate with managers on project progress
  • Develops and supports positive and engaged team culture.
  • Provides oversight and support
  • Leads all aspects of practice transformation


  • Develop and deliver presentations and training in group setting
  • Educate practice on knowledge and implementation of metric specifications
  • Host on-site and off-site training of EHR systems (if needed)
  • Advise work flow implementations to achieve triple aim at the office level
  • Identify and provide updated material regarding changes in quality metrics
  • Organize and update tools utilized to share lists provided to the practices


  • Present ideas and concepts verbally and in writing in a way that fosters learning
  • Collaboration and transfer of skills
  • Support practice with needs by being their point of contact. 



  • High School Diploma required- college education preferred

Skill(s)Basic PC skills (MS Word/Excel, Adobe Reader)


  • Centricity EMR, IDX, CareManager, HealthManager (training will be provided)
  • Data analysis skills
  • Highly developed interpersonal skills including motivational interviewing skills, in order to interact effectively and motivate practices to change behavior when necessary
  • Demonstrated strong writing skills; excellent oral communication and presentation skills
  • Ability to function as an integrated member of a team, take direction and follow-up
  • Excellent computer literacy and an ability to learn new computer skills and systems
