    Continuum Health is seeking a motivated individual to join our dynamic team.  The position of Patient Care Coordinator (PCC) is a nursing position in our Population Health Management group.  The PCC works with a multidisciplinary team, in a centralized location, managing complex patients in order to help achieve improved patient health, increased satisfaction, and lower cost of care.  We are looking for an individual who is able to work at the top of their license, be self-directed, and very organized.


    • Candidate must be a nurse registered in the state of NJ
    • Ideal candidate will have 2+ years of clinical experience as RN/LPN
    • Recent acute care, ambulatory, specialty, or home health experience is a must
    • Excel skills recommended
    • Excellent communication skills
    • Candidate must be able to execute workflows and work autonomously
    • Must have valid driver license and willing to travel
    • Must be bilingual in Spanish & English
