    General Summary: The Practice Transformation Coordinator will facilitate and lead practice  transformation with providers and office staff participating in value based programs.  Through a combination of on-site visits and remote (webex, phone) contacts, the Coordinator will support practices to re-design workflows that allow the practice to implement population health best practices, improve patient experience, quality of care, and lower cost of care.

    Responsibilities

     

    1. Project Management
      • Leads all aspects of practice transformation activities between insurance programs and the physician practices
      • Develop, maintain and manage improvement initiatives with multiple practices
      • Facilitate practice meetings with multidisciplinary clinical and non-clinical members
      • Proactively manage issues and risks to provide a high level client satisfaction  
      • Communicate with internal and external leaders on project progress or issues
      • Develop and support patient centered culture
    2. Education / Implementation
      • Develops and presents material to individuals and/or in a group setting aimed at delivering high quality care within total medical cost thresholds
      • Educate practice on knowledge and implementation of quality measures (ie HEDIS, NCQA)
      • Advise and collaborate with office on work flow implementations to achieve Triple Aim – Improving patient experience, improving health of the population, reducing per capita cost of care - at the office level
      • Organize, create and update performance reports to support transformation initiatives
      • Knowledge, understanding or willingness to learn the framework of problem solving and improvement methodology 
    3. Communication
      • Present ideas and concepts verbally and in writing in a way that fosters learning
      • Client focused and collaborative nature – ability to identify and meet needs of clients 
      • Effectively communicate as the liaison between the insurance payer and the provider/practice

    Qualifications

     

    Education

    • High School Diploma required
    • College education preferred (would consider extensive medical setting experience in lieu of college degree)   

     

     

     

    Skill(s):  Basic PC skills (MS Word/Excel, Adobe Reader)

    • Highly developed interpersonal skills including motivational skills, in order to interact effectively and motivate practices to change behavior when necessary
    • Data analysis skills – ability to identify trends and opportunities
    • Demonstrated strong writing skills; excellent oral communication and presentation skills
    • Ability to function as an integrated member of a team, take direction and follow-up
    • Excellent computer literacy and an ability to learn new computer skills and systems
    • Centricity EMR, IDX, Tableau (training will be provided)

     

    Experience:

    • Experience working in a fast paced environment communicating with multiple stakeholders
    • Healthcare setting preferred
