General Summary: The Practice Transformation Coordinator will facilitate and lead practice transformation with providers and office staff participating in value based programs. Through a combination of on-site visits and remote (webex, phone) contacts, the Coordinator will support practices to re-design workflows that allow the practice to implement population health best practices, improve patient experience, quality of care, and lower cost of care.
